4.1 Required Elements of a Valid Counter‑Notice

Your counter-notice must include:

  1. Your name, address, and contact information.

  2. Identification of the content removed and its location prior to removal.

  3. A statement under penalty of perjury that you believe the content was removed by mistake.

  4. A statement consenting to the jurisdiction of the courts in your region.

  5. Your physical or electronic signature.
